The Structure House program is unlike any other in the world and our 30 year history of success attests to this fact. Every participant can develop a specialized program uniquely developed to meet your specific goal. Structure House recommends the four-week intensive Core Program for new participants although, if necessary, you can also choose a one, two, or three week program. Fees can range from approximately $3,206 for a one-week program to $9,725 for the four-week program.

Your program will include individually selected meals, evaluative sessions with our professional staff, use of all facilities, medical and fitness testing, plus a schedule of more than 90 classes and activities per week.

New Participant rates apply for the first four weeks of your stay. Although the best benefits will be received from attending all four weeks together, the program may be attended in separate weeks of one, two, or three week intervals as needed. If you are unable to complete the four weeks consecutively you may chose to pay for the full four week program upon arrival yet complete your four weeks over the course of a year. This allows you to take advantage of the four week rate without having to complete them consecutively. You can also choose to participate in a one, two, or three week program.

Programs for new participants begin every week. Plan to arrive on Sunday for apartment check-in and Sunday dinner. Orientation begins each Monday at 7:45 a.m.

Simply call 800-553-0052 to speak with a participant representative. A $500 deposit will be taken to secure your reservation. We will send you a confirmation packet once we receive your deposit. The deposit is refundable less a $50 processing fee with a two-week notice of cancellation.

Structure House is a private pay facility. A $500 deposit is required at the time of reservation and the balance of New Program fees is due at check in. Structure House accepts personal checks, Master Card and Visa.

Portions of your stay at Structure House may be reimbursable by your insurance company; please contact your insurance company directly for reimbursement information. Your stay may also be eligible for payment through your medical spending account or may be tax deductible. Contact your benefits administrator and your tax advisor for further information. Structure House is not a Medicare Service Provider.

A 4% service charge is added to all program fees in lieu of gratuities. This service charge is distributed in its entirety directly to the Structure House staff.

High Season is generally February through October. Low Season is generally November through January.

DIABETES PROGRAM



Structure House offers a Diabetes Program in conjunction with our New Participant program providing state of the art medical care and lifestyle change for Structure House participants who have Type II diabetes. Rates range from an additional $600 for a one-week program to an additional $1,200 for the four-week program.

FAMILY SUPPORT PROGRAM



If you wish to bring a loved one or companion to Structure House with you, they may stay in your apartment and have a choice of two different levels of participation. The first, for $1,095 a week allows your support person to attend most classes and workshops, utilize the fitness facilities and swimming pools, and eat all meals in the dining room.

The second, for $795 allows your companion to utilize the fitness facilities and eat all meals in the dining room. It does not permit attendance at classes or workshops.

TAX DEDUCTIBILITY



Weight-Loss Programs May Be Tax Deductible.

On April 2, 2002, the Internal Revenue Service announced a new policy (IRS Ruling 202-19). Some overweight Americans may now be eligible to deduct the cost of weight-loss programs. You may be able to deduct qualifying expenses as part of medical care.

You may also be eligible for three employee benefits: Medical Savings Accounts, Flexible Spending Accounts, and Health Reimbursement expenses based on a doctor's recommendation.

Please consult your tax advisor and/or your employer regarding your individual circumstance.

TALK TO YOUR DOCTOR



Talk to your doctor about your BMI and how it relates to your health. If he or she advises you to lose weight to improve your health status, be sure to ask for a letter or referral. This letter can be essential If you plan to deduct weight-loss treatment costs on your tax return, or if you plan to submit them to your Medical Savings Account or Flexible Savings Account for reimbursement.

INSURANCE COVERAGE



Structure House accepts private pay only; however, you should check with your insurance carrier to see if reimbursement is available. Tips regarding insurance reimbursement are available from the American Obesity Association.
