The Diabetes Module is an optional add-on track available to any Structure House participant with Type 2 Diabetes, pre-Diabetes, or impaired fasting glucose or insulin resistance. Individuals can self-enroll in the Diabetes Module (no physician referral required), or we are happy to accept a referral from a provider and will coordinate communication with existing diabetes providers before, during, and after treatment as needed and desired.
We developed the Diabetes Module because we understand how challenging this disease can be to manage, in part because so much of diabetes treatment is lifestyle-based, including modifying eating patterns and increasing daily physical activity. The Diabetes Module is intended to help individuals understand factors affecting their glucose control, achieve and maintain glucose stability, address barriers to diabetes self-management adherence, and identify optimal medication regimens. Participants enrolled in our Diabetes Module receive the complete Structure House Core Program for healthy behavioral weight management while also engaging in specialized individual appointments with our consulting endocrinologist, Registered Nurse, and Registered Dietitian to improve glucose control and learn how to manage diabetes effectively at home.
Published results from our 4-week Diabetes Module show meaningful improvements across several indicators of diabetes and cardiovascular health, including reductions in fasting glucose, hemoglobin A1c, cholesterol, triglycerides, and body mass index. Additionally, nearly two-thirds of participants were able to reduce or eliminate at least one diabetes medication after four weeks of treatment (Pells JJ, Stout AL, Rodriguez-Diaz MM, Quigley CA, Musante GJ. In addition to participating in all of the individual assessments and appointments, behavioral, nutrition, and health classes, support groups, fresh nutritious meals, and fitness opportunities that are part of the Core Program, the Diabetes Module includes:
- Weekly medical appointments with a board-certified Endocrinologist including comprehensive assessment of diabetes history, medications, and lifestyle factors, as well as careful monitoring of glucose levels and medication needs
- Individual Diabetes Care session (Week 1) and Healthy Living with Diabetes classes (Week 2, 3,4) with Certified Diabetes Educator/Registered Nurse
- Weekly extra individual nutrition counseling with Registered Dietitian focused on optimal diabetes management through nutrition both at Structure House and at home
- Additional assistance as needed with Registered Nurse for extra help with glucose monitoring, preventing hypoglycemia, foot checks, or other diabetes-related needs
